Save the file and restart Gemini CLI.

Ask for something extract_design_md does. Gemini CLI lists the server's tools on connect and calls extract_design_md itself — you do not invoke it by name.

If nothing happens, check the server is running and that DESIGN.md Linter & Extractor's identifier in your config matches the one above exactly.
