Save the file and restart ChatGPT.

Ask for something usage_and_upgrade does. ChatGPT lists the server's tools on connect and calls usage_and_upgrade itself — you do not invoke it by name.

If nothing happens, check the server is running and that Approval Rules's identifier in your config matches the one above exactly.
