Save the file and restart ChatGPT.

Ask for something get_failure_mode does. ChatGPT lists the server's tools on connect and calls get_failure_mode itself — you do not invoke it by name.

If nothing happens, check the server is running and that failmodes's identifier in your config matches the one above exactly.
