Save the file and restart Claude Desktop.

Ask for something approve_milestones does. Claude Desktop lists the server's tools on connect and calls approve_milestones itself — you do not invoke it by name.

If nothing happens, check the server is running and that Milestone by Ouroboros Apps's identifier in your config matches the one above exactly.
