Save the file and restart VS Code.

Ask for something save_work_item does. VS Code lists the server's tools on connect and calls save_work_item itself — you do not invoke it by name.

If nothing happens, check the server is running and that Milestone by Ouroboros Apps's identifier in your config matches the one above exactly.
