Save the file and restart VS Code.

Ask for something report_blocker_outcome does. VS Code lists the server's tools on connect and calls report_blocker_outcome itself — you do not invoke it by name.

If nothing happens, check the server is running and that AgentWork Outcome Router's identifier in your config matches the one above exactly.
