Save the file and restart Cursor.

Ask for something create_patient_stream does. Cursor lists the server's tools on connect and calls create_patient_stream itself — you do not invoke it by name.

If nothing happens, check the server is running and that Uphealth Signal's identifier in your config matches the one above exactly.
